1. Go to the fillable pdf form, or the word doc for DE-147 - Duties and Liabilities of Personal Representative , at the L.A. Superior Court - see http://www.courtinfo.ca.gov/forms/fillable/de147.pdf or the word doc at http://profj.us/24su/law20/de-forms/de147.doc - and then you can attach the word doc to your moodle classroom assignment.

2. Then take the following facts - Facts to use with all Probate Forms and use those facts and information to fill in the Petition. If you do not use the word doc, then fill in the pdf form, print it out, and fax it to your online instructor.. You cannot save the information you input to a pdf file in pdf format, so make sure you print it out and fax it. 

3. If you want to keep a record that you faxed it to your online instructor, you can attach a word doc or text file to the assignment, showing the date and time you faxed it to your online instructor so he/she can look for it.
